Out of State Fingerprint Services
For clients out of Illinois state, fingerprint card scanning services
are available at our locations.
Please follow the instructions mentioned below:
1] Please call us at 312-782-8143
or email fingerprintingchicago@gmail.com
and we shall provide fingerprint card.
2] Please fill out the card as per the instructions provided on the
card and get fingerprinted (BLACK INK) from local law
enforcement agency or any authorized private fingerprinting
vendor.
3] Also mention the reason for being fingerprinted such as:
Reason for Inquiry Purpose Code ORI
Registered Nurse RPN - 041 IL920630Z
Security Guards SEC - 129 IL920020Z
Message Therapist MTH - 227 IL920680Z
Or any other reason for inquiry.
4] Mail the completed card and $ 55 money order payable to
"A Fingerprinting U S Photo" with your name, address, telephone
number and email address to:

Chicago, IL - 60603
5] After receiving your card and payment, we shall process the
request immediately and transfer the card electronically to Illinois
State Police for background check and the results will be
forwarded to the appropriate agency.
6] After the card submission electronically, the generated TCN
(Transaction Control Number) will be sent to you by Email. Use
this TCN number to submit your application back to Illinois State
agency that issues the license.
